Want to Participate as a DASA Athlete?

If you are an individual with a physical disability, a family member, or a caregiver who is interested in athletic and rehabilitative programs for yourself or a loved one, and you live near St. Louis or Columbia, Missouri, you’re in the right place. The Disabled Athlete Sports Association helps physically disabled, hearing and/or vision-impaired individuals live healthy, active lives through fitness, sports, and athletics. We welcome you to participate and become a DASA athlete!

Who is Eligible to Participate?

DASA works with cognitively age-appropriate individuals 5 years of age and older, including military veterans and injured first responders, living with permanent physical, visual, and hearing disabilities who have difficulty leading an independent life and participating in sports with their non-disabled peers.

DASA empowers people of all ages who have:

  • Cerebral Palsy
  • Spinal Cord Injury
  • Spina Bifida
  • Muscular Dystrophy
  • Amputation
  • Deaf or Hard of Hearing
  • Visual Impairment
  • Neuromuscular/Orthopedic Conditions

As part of the DASA family, we know you will discover new abilities and confidence through sports and adaptive training. Whether you’re interested in rehabilitation, athletics, or both, we are here to help you achieve your goals and live your dreams. Simply start by following the new athlete form instructions below.

Start with the New Athlete Form

If you want to participate in DASA programs, we recommend you have all the necessary information on hand before you start the application – once you start, if you back out of the application, the information will be lost and you’ll have to start again. Info needed: in addition to basic identification like name, address, date of birth, and phone numbers, we need to know your Department of Mental Health (DMH) number (if applicable), case numbers, all doctors’ and therapists’ information, emergency contacts’ names and phone numbers, and employer’s information. We also ask you to attach a digital photo.
Once you start the New Athlete Form, you MUST fill it out completely prior to participating at DASA. Just click the button, fill in all the information, and we will contact you as soon as we can to get started.
If you have questions or difficulty filling out the form, contact us here, and one of our staff will help you with the process.
To participate in a DASA program/event, a Liability Waiver (Part 1) and Media Release (Part 2) MUST be signed. Simply click the “DASA WAIVER” button or use the link that’s provided near the end of the New Athlete Form.
